Register with our surgery
To register with our surgery please complete an Online Patient Registration Form (for both adults and children) and a New Patient Questionnaire (required for adults only).
These can also be obtained from our reception team or printed from the below links, please bring any forms printed and completed to the surgery. We don’t need to see ID from patients, but we may ask for supporting documentation as it can help the surgery find your medical records or transfer them from your current GP required. If you do not have a permanent address you can still register using a temporary address.
Registration at The Private GP clinic will not affect your registration with NHS services, and indeed we encourage all our patients to remain registered with their NHS GP.
Private GP Services
We offer prompt, longer appointments for both adults and children so you have plenty of time to tell us about your problem. Appointments can be telephonic or face to face and we are able to prescribe and refer to specialists if needed.

Our male or female GP can work with you to make sense of and manage your wellbeing. We can liaise directly with other doctors and practitioners to help you understand your conditions. This is only done on your request and with your consent.

In Clinic
Our GP consultations can be made for any healthcare need you have, and are for patients of all ages. Our standard GP consultation is 20 minutes in duration, allowing plenty of time to explore your concerns in detail. You can also book extended appointments of 40 minutes if you feel you need more time, or have multiple issues to discuss. We aim to provide as much time as you need.
